This form is used to identify each person notified regarding the disposal of a vehicle valued at $500 or less impounded by a public agency. Side A is for vehicles removed by a public agency for reasons other than abandonment CVC 22851.8, and side B is for vehicles removed by a public agency because of abandonment CVC 22851.8.
